This assessment will ask a series of questions that will help you obtain a snapshot of where you stand in a variety of important areas in your life. Please know in advance that we recognize the sensitive nature to some of the questions and take very seriously our responsibility to maintain your confidentiality. Your assessment will be between you and Frame of Mind Coaching and no one else!
For questions that ask to rate yourself on a scale of 1-10, 1 is not at all and 10 is all the time. We encourage you to request a review of your results with a coach for a better understanding of both the questions and how to make changes in your life to improve your scores.
